SAFETY STATEMENT
Craniosacral therpay is provided here by (therapists name) and is delivered through clothing. Children under 18 years of age must be accompanied by a parent or gaurdian. There are limits to confidentiality regarding concerns about children which reach a particular abuse threshold to Tusla, retrospective reporting of child abuse and potential suicide.
GENERAL DATE PROTECTION REGULATIONS (GDPR)
Signed Consent
It is suggested that therapy clients sign their consent to data holding along with consent to treatment on their initial intake form.
Example:
"I agree to have my data held by (therapist Name) for the purpose of proceeding with my treatments only and understand that it will be held in accordance with the GDPR 2018 which have been explained to me. "
BACK DATING CONSENT
Seek consent on the next visit, where file on client preceded 25th May 2018.
Remember to:
Destroy all files after the period of retention 8 years after last visit for adults AND 8 years following their 18th birthday for children.
DIGITAL CONSENT
If you have any digital marketing or booking systmes, there are very strict rules.
Individuals have the right to apt out by default. Any tick-box option needs to be giving consent not withholding consent.
They should be able to opt out at any later date quickly and easily, if they choose to.
